Registered parishioners of Saint Catherine Labouré Parish who need a Sponsor Certificate for Baptism or Confirmation should complete the request form using the link below.
Alternatively, outside sponsors may complete the form, print it, and have it approved by their own parish, then forward the certificate to St. Catherine Labouré Parish, 4000 Derry St., Harrisburg, PA 17111.
After submitting the form online, please print, sign, and bring it to the parish office for review and the Pastor’s signature.
Submit the completed form at least two weeks before the date the certificate is needed.
Sponsors from other parishes should contact their own parish to obtain a sponsor letter.
